Just a plan I found online today as I get back into the swing of things and prepare for multiple 504 meetings in upcoming weeks . . .

Some of the wording and organization may be useful to some here, IMHO.

Although I was not thrilled with the homepage from which this came and the (incorrect) use of "reasonable accommodations"  vs.  504's "necessary and appropriate" benchmark for accommodations in public school setting:

An additional comment as to the IBD student's needs -- note that there is accommodation to carry/eat food at any time (to include classroom) -- remember that the LTFA child's needs would need to be deconflicted with the IBD or diabetic child's (or teacher's) medically necessary accommodations as well.

Just a thought.
