Never had to fill this form out for school before

Physician's Statement for Students with Special Dietary Needs

Holden's had both allergy and asthma plans at school since he was in 1st grade (so primary and intermediate school).

He's now in 6th grade in middle school. Any reason as to why we are just NOW being asked to have this filleed out?


NEVER have we had to have a doctor fill out THIS form. Wondering if I should be pushing for a 504 with this or not. *I* think he could probably use a 504 for his ADHD issues and the allergy issues could certainly fall into this as well.

The new(?) school might be better at performing its Child Find duties.

This is the first time I've had this form, too (I posted about it in my thread on district forms in OT). I have seen it being used this year in school districts around the country. It may be new everywhere.

I love it!

Ours refers to special dietary needs because if a disability.  It made me realize that our district is wiser to 504s for FAs than I has thought last year.
Me: Sesame, shellfish, chamomile, sage
DS: Peanuts

so does this one.

He's not marked as having a disability but it DOES state he has special nutritional or feeding needs and then states that avoidance of cross contamination of instruments, oils, etc is to be adhered to.

I'm assuming this was more for a student that will be taking teen living (he's not as he's in orchestra which takes the place of his exploratory electives) but it's still kind of neat to see.

So who do I turn this form into? The office, school nurse or cafeteria?

I took ours to our 504 tweaking meeting and eventually handed it to the cafeteria manager.

But our a school already had him categorized as having a disability because of the 504.  I do think this form could be a nice step in getting a 504.

For our school, it is for cafeteria food. By law, school cafeterias have to provide a safe lunch. This form helps them meet that need. Many of us for years before the new ADAAA used the USDA guidelines to help build the case for a 504--to help connect those dots for schools. Again, the USDA is really helping our kids with this form (i assume it came from them). It's very cool.
