I took ours to our 504 tweaking meeting and eventually handed it to the cafeteria manager.
But our a school already had him categorized as having a disability because of the 504. I do think this form could be a nice step in getting a 504.
For our school, it is for cafeteria food. By law, school cafeterias have to provide a safe lunch. This form helps them meet that need. Many of us for years before the new ADAAA used the USDA guidelines to help build the case for a 504--to help connect those dots for schools. Again, the USDA is really helping our kids with this form (i assume it came from them). It's very cool.
